Registration Forms

The forms below must be completed and returned electronically

prior to joining the team on the ice.

All players, on-ice volunteers, and coaches must be registered with both ASHA and USA Hockey. USA Hockey registration is free to first-time players (the coaching staff will register first-time players).

In addition, anyone (player, on-ice volunteer, coach) who will be 18 years of age or older as of December 31st must complete the NCIS Background Screen, as well as Abuse Prevention Training, to be in compliance with the Federal Safe Sport Act Regulations.
